Participation Agreement

What this is

Re-Membered Body Work and Sound offers body work sessions and sound sessions. Jules holds a practitioner certificate in body work. She is not a licensed health provider, and nothing offered here is a medical service, a substitute for one, or a way to identify or address any medical condition.

Body work in this space is slow, hands-on work with the forearm, elbow, hands and thumbs, meant to bring held tissue back into connection and movement. Sound sessions use singing bowls, a gong, drums, chimes and similar instruments while you lie or sit still. Every body work session includes an essential oil application to the skin.

What I am telling you

I am eighteen years of age or older, or I am the parent or legal guardian of the participant and I am signing for them.

I have told Jules, in writing below or in conversation, about any injury, surgery, pregnancy, skin sensitivity or allergy, medication that affects bleeding or bruising, bone fragility, or condition my physician is actively managing. I understand I should talk with my physician before body work if any of those apply, and that a sound session may be the better first step.

I will tell Jules during a session if anything feels wrong, and I understand I can ask for less pressure or stop at any time, for any reason.

What I understand

Body work can feel intense where tissue is held, and some tenderness for a day or two afterward is common. Sound sessions involve lying still for up to an hour; some people fall asleep and some feel restless, and both are normal.

Results are not promised. What other people describe after a session is their experience, not a guarantee of mine.

Sessions are paid at booking. Twenty-four hours notice is needed to move or cancel a session, and a session cancelled inside that window is not refunded.

Assumption of risk and release

I choose to take part voluntarily. I accept the ordinary risks of hands-on body work and of lying still during a sound session, including soreness, bruising, a skin reaction to an essential oil, or an aggravation of a condition I did not disclose.

To the fullest extent allowed by Florida law, I release Re-Membered Body Work and Sound, Jules, and anyone working with her from claims arising from my participation, except for claims arising from gross negligence or intentional misconduct. This release covers today and every later session unless I withdraw it in writing.

Signing electronically

By checking the boxes and typing my full legal name below, I am signing this agreement electronically, and I agree that my electronic signature has the same effect as a signature on paper. A copy is sent to Re-Membered when I sign, and I can print a copy for myself.
